been prepared as directed by the Commission in its orders in Case No. U-12487,
dated June
5, 2001 and December
20, 2001, concerning customer disclosure standards as required under
Section 10r(1) of Act 141 PA 2000. Suppliers may voluntarily download and
use this template, linked below in an EXCEL spreadsheet format, or may develop
their own template as indicated in the Order. |
