Instructions for Completing the Application for Standard Well Connection Permit Form using Microsoft Word

This form was created in Microsoft Word 97 and saved in a Word 97-2000 & 6.0/95-RTF (*doc) format. The document is too heavily formatted to open in Microsoft WordPad. While the file is opening, if Word displays a warning that this file contains macros, choose the "Enable Macros" option.

Once the form is open, you should be able to view all but the rightmost inch of the form's width.

Use your mouse to place the cursor in the first data entry box labeled "Operator". After the operator name is entered, use the Tab key to move to the next data entry field. If you need to move to a data entry box that is not next in the Tab sequence, use your mouse to click in the desired data entry box . If the Enter key is used, a new row in the table will be created. Only use the Enter key in certain data entry boxes as described below. Use the Undo feature (Edit Menu/Undo) to remove unwanted rows created by the Enter key.

Box 19 - Location
Press the Space Bar, click your mouse or type an "X" while in the checkbox if the bottom hole location is the same as the surface hole location. To leave the checkbox blank, use the Tab key to move to the next data entry field.

Box 20 - Well Location Plot
The well location can be plotted by hand after the form is printed. It can also be plotted by using the mouse to place the cursor at the left edge of the diagram at the desired height and using the space bar to move to the right. When the cursor is at the correct well location, mark the spot with an "X". Use the mouse to move to the next data entry field to continue filling out the form.

Box 21 & 22 - Drilling Permit Number and Operator
A macro is used to automatically fill out these boxes.

Box 35 - Type of Completion
Press the Enter key if more than one row is necessary.

The form can be saved with the data you entered. Use the Save As option and save the file under a new name.

Print the form and include a signature and date. . When printing to a Laserjet printer, the form is set up to print duplexed with the back page fed manually. To print the front and back sides on separate pages choose Page Setup from the File menu and then go to Paper Source.
