| Instructions for Completing the Application for Standard Well Connection Permit Form using Microsoft Word
This form was created in Microsoft Word 97 and saved in a Word 97-2000 & 6.0/95-RTF (*doc) format. The document is too heavily formatted to open in Microsoft WordPad. While the file is opening, if Word displays a warning that this file contains macros, choose the "Enable Macros" option. Once the form is open, you should be able to view all but the rightmost inch of the form's width. Use your mouse to place the cursor in the first data entry box labeled "Operator". After the operator name is entered, use the Tab key to move to the next data entry field. If you need to move to a data entry box that is not next in the Tab sequence, use your mouse to click in the desired data entry box . If the Enter key is used, a new row in the table will be created. Only use the Enter key in certain data entry boxes as described below. Use the Undo feature (Edit Menu/Undo) to remove unwanted rows created by the Enter key. Use the Save As option and save the file under a new name. Print the form and include a signature and date. . When printing to a Laserjet printer, the form is set up to print duplexed with the back page fed manually. To print the front and back sides on separate pages choose Page Setup from the File menu and then go to Paper Source. Follow the mailing instructions on the bottom of the second page.
