Instructions for Completing the Application for Standard Well Connection Permit Form using Microsoft Excel

This form was created in Microsoft Excel 97 and saved in Excel 97-2000 & 5.0/95 Workbook (*xls) format. The Excel file is set up with two sheets. Sheets P171GF and P171GB represent the front and back of the application form.

Box 1 - Operator
Use your mouse to place the cursor in cell B22 in the "Operator" data entry box. All other cells in the box are protected and will not accept data. After the operator name is entered, use the mouse or arrow keys to move to the next data entry field. The operator name is automatically filled in on the back page of the document.

Box 16 - Drilling Permit Number
Use your mouse to place the cursor in cell B38 in the "Drilling Permit Number" data entry box. All other cells in the box are protected and will not accept data. The drilling permit number is automatically filled in on the back page of the document.

Box 20 - Well Location Plot
The well location can be plotted by hand after the form is printed. It can also be plotted in Microsoft Excel by marking the spot with an "X".

Box 21 & 22 - Drilling Permit Number and Operator
These boxes are automatically filled out using information entered on the first page of the form.

The form can be saved with the data you entered. Use the Save As option and save the file under a new name.

Print the form and include a signature and date. Follow the mailing instructions on the bottom of the second page.
